  • What does a first-time client need to bring to their initial appointment?

    When coming to C&A for your in-take appointment, you will need your child’s:


    • Birth certificate
    • Social security card
    • Insurance card
    • Proof of income (last pay stub)
    • If applicable to the household, custody papers
  • What types of insurance do you accept?

    Public Aid and most insurance are accepted.

  • Do you have co-pays?

    If your insurance requires a co-pay, that must be paid at the time of the appointment. A co-pay payment may be made with check or credit card

  • Do you have evening appointments?

    Yes, we offer evening appointments. C & A offers limited evening hours. These are our most requested appointments.

  • Do you have Saturday appointments?

    No, we do not offer Saturday appointments

  • Do we have support groups or offer group activities?

    Yes, we do have support groups

  • Do you need a referral for C&A services?

    No, you do not need a referral

  • Do you accept walk-in appointments?

    No

  • How long is the wait list for an appointment?

    We typically can see a new client within 10 business days

  • What age group do we serve?

    We serve children 0 to 24-years-old

  • What is an intake appointment?

    There are two parts to an intake appointment. The first part of the intake appointment is filling out forms and gathering patient information.


    The second part of an intake appointment is an assessment to see which service best meets the client's needs.
